Privacy
How Kiwi Orthotics handles the information you give us through this website.
Your information
Kiwi Orthotics collects and holds health information about you so we can assess, treat and review your orthotic care. We handle it under the Privacy Act 2020 and the Health Information Privacy Code 2020.
What we collect
Your name, date of birth, contact details and NHI number; the reason you were referred and who referred you; clinical notes, measurements, scans, photographs and casts taken during your care; the devices supplied to you; and funding or claim details where ACC, Health NZ or an insurer is involved.
What the website collects
Much less. The referral form asks for the patient’s name, date of birth, a contact number, town and gender, the funding source, an ACC claim number and date of injury where ACC applies, and a brief reason for referral. If someone is referring on a patient’s behalf we also ask for their name, role, practice and contact email.
Date of birth together with a reason for referral is health information about an identifiable person, which is why we keep the form short. It is for making contact, not for transferring clinical records. Submissions are delivered to our email and are not stored by the website. Our email is held in Microsoft’s New Zealand data centres under a business agreement, so the only lasting copy of your referral stays in New Zealand. If you would rather not use the form, phone or email us instead.
Why we collect it
To contact you, arrange an appointment, confirm which funding pathway applies, provide and review your orthotic treatment, and write back to the person who referred you.
Who else sees it
The clinician who referred you, and ACC, Health NZ or your insurer where they are funding your treatment. Where a device is made by a supplier, we share only the measurements and specifications needed to make it. Our email and records are held in Microsoft 365, under a business agreement, with data stored in New Zealand. We do not sell your information and we do not use it for marketing.
How long we keep it
Health records are held for at least ten years from your last appointment, as New Zealand law requires. Website enquiries that do not become appointments are deleted within twelve months. Financial records are held for seven years for tax purposes.
